The Community Hospice provides short-term therapeutic services to facilitate successful transitions through the grief process. We offer support through a variety of modalities, including individual, family, and group counseling. Grief counseling is available for individuals of any age, or families, and is free of charge. COVID-19 Update

We at The Community Hospice are thinking of you all during this extraordinarily difficult and isolating time. We recognize that there is a urgent need for grief support and we are working harder than ever to meet that need. We are offering both individual and group support through phone or video meetings at this time. All services are free of charge.

Someone will return your call to explore your needs and assess the best course of treatment for you. Once you have been connected with a clinician, you will be emailed/mailed all necessary paperwork. You are also welcome to download the forms from our website.
